How do I schedule an appointment with a provider? What should I say when I call?
In an effort to provide you with meaningful savings on a variety of services, Reassurance Health has made arrangements with a variety of different healthcare networks that negotiate discounts with providers all over the U.S. These networks are listed on your membership card so you can access the discounts as part of this membership plan.When you make an appointment, or arrive at a participating provider, be sure to point out the networks on your card so the provider can give you your discount.
For example, your dental network is called UNI-CARE; your medical network is called Beech Street. For questions about other networks, contact Customer Service at 1-866-440-0528.
