The following limited insurance benefits are under the Group Accidental Death and Dismemberment and Medical Care Insurance policy and is an added benefit of your membership featuring:
$1,000.00Accident Medical/Dental Expense Reimbursement Pays part of the expenses you are charged by a hospital, doctor, or certain other charges, up to a maximum of the amount listed if you are injured in a covered accident. Subject to a $100 deductible.
$100.00Daily Hospital Confinement Benefit* If you are under age 65, it pays you the amount listed per day for up to 180 days if you have to be admitted and stay in a hospital to treat you for a covered injury or sickness.
$35.00 per visit 2 visits per person 4 visits per family per calendar yearDoctor Visits* If you are under age 65, it pays you the amount listed per visit to a doctor's office to be diagnosed or treated for a covered injury or sickness. You will be paid for up to 2 visits for individual coverage or 4 visits for family coverage per year.
$50.00 per visit 3 visits per person per yearEmergency Room* If you are under age 65, it pays you the amount listed each time it is necessary for you or your covered dependents (if this is a family membership) to get treatment for a covered injury or sickness by visiting the Emergency Room of a hospital. These plans include insurance and non-insurance benefits. This is not a Medicare prescription drug plan. These plans provide discounts at certain health care providers for medical services. Some plans also include fixed schedule insurance benefits. These plans do not make payments directly to the providers of medical services. The plan member is obligated to pay for all health care services but will receive a discount from those health care providers who have contracted with the discount medical plan organization, as well as reimbursement from certain plans. The range of discounts for services provided will vary depending on the provider type and services provided.
